How do I know if my testosterone is low?
Low testosterone is diagnosed with a morning blood test confirmed on a second draw, combined with symptoms. A number alone is not enough, and neither are symptoms alone.
The symptoms overlap with a lot of other things. Fatigue, low motivation, muscle loss despite training, weight around the middle, poor sleep, low libido, and erectile problems all show up on the list. So do sleep apnea, depression, thyroid disease, and poorly controlled blood sugar.
That is why the evaluation matters more than the prescription. Treating low testosterone when the real problem is untreated sleep apnea does not fix the sleep apnea.
What we do
What the workup includes.
- Total and free testosterone, drawn correctly, in the morning and confirmed on a second sample.
- The rest of the hormone picture. LH, FSH, prolactin, estradiol, and SHBG, because they tell us where the problem is coming from.
- Metabolic and cardiovascular screening. Insulin resistance and visceral fat lower testosterone directly.
- Sleep evaluation. Untreated sleep apnea suppresses testosterone and is badly underdiagnosed in men who train and look healthy.
- Body composition by DEXA, so we are tracking muscle and visceral fat rather than a number on a scale.
- Fertility discussed up front. Testosterone therapy suppresses sperm production. If you may want children, that changes the plan and there are alternatives.
Is testosterone replacement safe?
For appropriately selected and properly monitored men, testosterone therapy is generally safe, but it requires ongoing monitoring and it is not right for everyone.
Monitoring is not optional. Hematocrit, PSA, estradiol, and symptom response all need tracking on a schedule, and that follow up is built into how we manage it.
There are also alternatives worth discussing first, particularly if you want to preserve fertility or your levels are borderline. Sometimes treating the underlying cause raises testosterone without replacing it at all.

How much does this cost?
DPC membership is $150 per month and is HSA and FSA eligible.
That covers unlimited visits, 30 to 60 minute appointments, and direct messaging with your care team. Lab work and any medication are separate and reviewed with you before anything is ordered.
Do I need a referral?
No. You can schedule directly.

Will testosterone therapy affect my fertility?
Yes. Testosterone therapy suppresses sperm production, and in some men that does not fully reverse.
If you may want children, say so at the first visit. There are treatment approaches that raise testosterone without suppressing fertility, and they are worth discussing before you start anything.
How often do I need bloodwork once I start treatment?
Typically at six to eight weeks after starting or changing a dose, then every six to twelve months once stable.
We monitor hematocrit, PSA, estradiol, and testosterone levels along with how you actually feel. Skipping monitoring is how testosterone therapy causes problems.
My levels came back normal but I feel terrible. What now?
Then testosterone probably is not the answer, and something else is.
Sleep apnea, insulin resistance, thyroid disease, and depression all produce the same symptom list. A full workup finds the actual cause instead of treating the one number that was easiest to check.
